Announcement of Commencement of Practice for Opticians - Optometrists

Application and submission of supporting documents for the issuance of a Certificate of Commencement of Practice for the profession of Optician - Optometrist.

Process: The steps for completing the process are as follows:

1. Application with the required supporting documents.

2. Issuance of the Certificate of Commencement of Practice.

Supporting Documents: For the Announcement of Commencement of Practice for Opticians - Optometrists, the interested party must submit:

  1. Application-Solemn Declaration for the announcement of commencement of professional practice.

  2. Solemn Declaration (digitally signed via GOV.GR) stating that: "I have not been irrevocably convicted of a felony, theft, embezzlement, fraud, extortion, forgery, counterfeiting, falsification, drug law violation, intentional homicide, infanticide, kidnapping, crimes against sexual freedom, crimes of economic exploitation of sexual life, or profiteering."

  3. Public Fee (Paravolo), amount of eight Euros (8 €), e-Paravolo, Code: 5015, according to the following instructions: (www.gsis.gr→ e-Paravolo → Issuance of Fee → Agency: Horizontal Fees → Fee Category: Licenses → Fee Type: [5015] License to practice a health profession).

  4. Two (2) identical recent passport-sized photographs (to be submitted to the Service).

  5. Photocopy of ID card or any identification document.

  6. Nationals of EU member states or third-country nationals with the rights of EU nationals must additionally submit a registration certificate for EU citizens, or a permanent residence certificate/European citizen card (Article 42 of Law 4071/2012), or an expatriate identity card, or an expatriate residence permit (Article 81 of Law 4251/2014), or a residence card for a family member of a Greek citizen, or a permanent residence card, or a personal residence permit for third-country nationals (Articles 82, 83, 84, 85, 87 of Law 4251/2014), or a long-term resident permit for third-country nationals (Articles 88, 97, 106 of Law 4251/2014), or a second-generation residence permit (Article 108 of Law 4251/2014), a copy of the EU Blue Card (Articles 114 and 120 of Law 4251/2014), or a copy of a ten-year residence permit (Article 138 of Law 4251/2014), or a family status certificate proving a cohabitation agreement with a Greek citizen (Article 12 of Law 4356/2015 and Article 3 of Law 4443/2016).

  7. Valid residence and work permit in Greece (applies to expatriates and foreigners of foreign origin and citizenship).

  8. Photocopy of University (A.E.I.) or Technical University (T.E.I.) degree or copy of a foreign degree (as provided) and a decision of professional equivalence or professional qualifications recognition issued by the Ministry of Education and Religious Affairs - Independent Department for the Implementation of European Legislation (A.T.E.E.N.), or a copy of a foreign degree (as provided) along with an act of equivalence and correspondence issued by the competent recognition authorities D.O.A.T.A.P. or I.T.E.

  9. Certificate of registration with the Panhellenic Association of Opticians & Optometrists (P.S.O.O.).

SUPPORTING DOCUMENTS FOR INTERNSHIPS FROM EU COUNTRIES, PURSUANT TO DIRECTIVES 92/51, 89/48, PRESIDENTIAL DECREES 231/98, 165/2000, AND MINISTERIAL DECISION NO. 89034/IA (FEK 2047/B’/14-09-2011).

  1. Application.

  2. Photocopy of the relevant decision of the S.A.E.P. (Council for the Recognition of Professional Qualifications) regarding the imposition of compensatory measures and the determination of the duration of the internship.

  3. Simple photocopy of the applicant's declaration to S.A.E.P. by which the compensatory measure of an adaptation internship was selected, in accordance with par. 2 of Article 14 of P.D. 38/2010.

  4. Solemn Declaration (Law 1599/86) from the entity where the internship takes place—specifically the person to whom the establishment and operation license has been issued or the head of the corresponding department of a nursing institution—signed and certified for the authenticity of the signature. The declaration must state the start date of the adaptation internship, the name of the supervisor, and the subject matter of the internship.

  5. Certification from the competent insurance body regarding the public or private insurance of the intern against the risk of accident.

  6. Photocopy of the store's operating license.

  7. Upon completion of the internship, a certificate of successful completion from the employer is submitted.
